Lifegate Youth Registration Form

Lifegate Youth takes place every Friday during the school term from 7-9 pm.

Lifegate Youth is for high school students. We believe in seeing youth live in the freedom and purpose that Jesus offers. We do this through creating environments for FUN, community, and creating a space where our youth feel welcomed and safe to explore life's questions.

Lifegate Youth experiences are posted each term on Facebook and Instagram. Please follow our pages to keep up to date with announcements.

(*This is to ensure your child's safety and so that we can verify they are leaving with the right people. If your child will not be picked up by one of the following drivers one week, please let Christie know on 0403 682 711.)

I give permission for my child to use public transport or walk to leave Lifegate Youth events. *

Terms and Conditions

By agreeing to the Terms and Conditions, you are agreeing to the following:

- I give permission for my child to attend Lifegate Youth in 2022.

- I understand that if urgent medical attention is needed, that every effort will be made to contact the persons listed on this form. In the event that I cannot be contacted in an emergency, I give my permission for my child to receive such medical treatment, as the leader may deem necessary. I will be responsible for any and all related expenses.

- I consent to information about my child being collected for incident forms when necessary.

- I give permission for my child to drive in a Lifegate Youth Leader's car to and from Lifegate Youth if I have not arranged alternate means of transportation.

- I give permission for my child to be transported in private cars, buses and/or public transport as arranged by the leaders for anything related to Lifegate Youth.

- I give permission for my child to walk to any locations deemed fit by Lifegate Youth Leaders with Lifegate Youth Leaders (e.g. the park for activities, local shops etc).

- I give Lifegate Church permission to take photographs and/or video of my child.

- I grant Lifegate Church full rights to use the images resulting from the photography/video filming and any reproductions or adaptations of the images for publicity or other purposes to help achieve the Church’s aims. This might include (but not limited to), the right to use them in printed and online publicity, social media, press releases and funding applications.

- I understand that all reasonable safety precautions will be taken at all times by Lifegate Church and that the leaders and those connected with the group cannot be held responsible for personal injury incurred by the child of this form.

- I understand that Lifegate Church cannot be held responsible for loss or damage of any items taken, by the child of this form, to Lifegate Youth programs.

- I acknowledge that this event will be held in accordance with any current NSW Health COVID-19 Public Health Orders and in accordance with Lifegate Church policies and procedures. I acknowledge and accept that there is a risk that my child may be exposed to COVID-19 whilst attending and participating in Lifegate Youth events. I confirm that my child will not attend if displaying any symptoms of illness and/or is directed to isolate under public health orders.

- I understand that Lifegate Church is a Christian Church where we teach on finding Freedom and Purpose through Jesus. I understand that Lifegate Youth programs will include teaching times where we will introduce them to Jesus and discuss topics related to and including the Bible.
